Contract Liaison

Role Summary

The independent contractor provides medically necessary, person-centered psychotherapy to nursing home residents within the contractor's licensure, competence, and professional judgment. Services may include diagnostic assessment, individual psychotherapy, family psychotherapy when clinically indicated, group psychotherapy when authorized, treatment planning, risk assessment, and interdisciplinary care coordination. The contractor practices independently and is responsible for the clinical means and methods used to deliver the contracted services.

Scope of Contracted Services

• Evaluate residents' behavioral health needs, functional context, strengths, preferences, psychosocial stressors, and treatment readiness.

• Develop, implement, and periodically update individualized psychotherapy treatment plans with measurable goals and clinically appropriate interventions.

• Provide evidence-informed psychotherapy appropriate to older adults, adults with disabilities, serious mental illness, trauma histories, substance use disorders, chronic medical illness, and cognitive impairment when the resident can meaningfully participate and benefit.

• Use approaches within competence, which may include supportive psychotherapy, cognitive behavioral strategies, motivational interviewing, behavioral activation, grief counseling, trauma-informed interventions, relapse-prevention strategies, and adjustment-focused therapy.

• Assess clinical change, barriers to treatment, response to interventions, and continued medical necessity; modify or conclude services when indicated.

• Screen for and respond to suicide risk, self-harm, violence risk, abuse, neglect, exploitation, acute psychiatric deterioration, and other urgent safety concerns in accordance with professional standards, facility protocols, and applicable law.

• Coordinate clinically relevant information with the resident, authorized representative, facility staff, primary care clinicians, psychiatric clinicians, social services, and other members of the interdisciplinary team, subject to privacy and consent requirements.

• Participate in care-planning discussions or clinically necessary case consultations when mutually scheduled and included within the contracted scope.

• Provide on-site and approved telehealth services consistent with payer rules, state licensure requirements, resident consent, privacy standards, and clinical appropriateness.

• Make appropriate referrals for psychiatric, medical, substance-use, neuropsychological, crisis, protective, or community services when needs exceed the psychotherapy scope.

Clinical Documentation and Compliance

• Complete accurate, timely, and individualized clinical documentation in the designated health record, including assessment findings, diagnosis when authorized, treatment goals, interventions, resident response, progress, risk findings, coordination, and follow-up plan.

• Document and bill only services personally rendered, medically necessary, supported by the record, and permitted by the contractor's license, payer enrollment, service agreement, and applicable law.

• Comply with HIPAA, 42 CFR Part 2 when applicable, professional ethics, mandatory-reporting duties, federal and state nursing home requirements relevant to the service, payer requirements, and reasonable facility health, safety, infection-control, emergency, and privacy procedures.

• Maintain professional boundaries, obtain informed consent as required, protect resident rights, and use culturally responsive, trauma-informed, and least-restrictive practices.

• Cooperate with lawful audits, quality reviews, and requests to correct incomplete or inaccurate documentation without altering the clinical record improperly.

Minimum Qualifications

• Master's or doctoral degree in social work, counseling, psychology, or a related behavioral health discipline from an appropriately accredited program.

• Current, unrestricted independent clinical license in each state where services are delivered, such as LCSW, LMHC, or LPCC, with authority to provide psychotherapy without employer-provided clinical supervision.

• Current professional liability insurance at the limits required by the independent contractor agreement and facility access requirements.

• Active individual National Provider Identifier and ability to satisfy applicable credentialing, exclusion-screening, background-check, health-screening, and payer-enrollment requirements.

• Knowledge of psychotherapy practice, risk assessment, clinical documentation, confidentiality, resident rights, and the behavioral health needs of nursing home residents.

• Ability to communicate effectively with residents who may have sensory, cognitive, neurological, psychiatric, or functional limitations.

• Reliable access to required technology, secure communications, and transportation for accepted assignments.

Preferred Experience

• Psychotherapy experience in skilled nursing, long-term care, geriatric, rehabilitation, serious mental illness, dementia-capable, palliative, or substance-use settings.

• Experience collaborating with interdisciplinary teams and documenting in electronic health records.

• Familiarity with Medicare, Medicaid, commercial payer, and telehealth documentation requirements applicable to psychotherapy services.

Independent Contractor Relationship

This description identifies the professional services that may be requested under a separate independent contractor agreement. It does not create an employment relationship, guarantee referrals, establish fixed hours, or require acceptance of any particular assignment. Subject to resident needs, accepted assignments, facility access requirements, and agreed service standards, the contractor controls the manner and means of clinical practice, exercises independent professional judgment, and determines availability and scheduling in coordination with the facility. The contractor is responsible for required licenses, continuing education, insurance, taxes, equipment, business expenses, and other obligations assigned by the written agreement and applicable law.

The contractor is not eligible for employee wages, overtime, paid leave, retirement benefits, unemployment benefits, workers' compensation benefits, or other employee benefits through the company, except to the extent a written agreement or controlling law expressly provides otherwise. The contractor may provide services to other organizations, subject to confidentiality, conflict-of-interest, non-solicitation, and other enforceable terms in the written agreement.

Professional Boundaries and Exclusions

• The contractor does not prescribe, administer, or independently change medications unless separately licensed and contracted to do so.

• The contractor does not replace nursing assessment, medical evaluation, emergency services, facility social services, MDS responsibilities, or legally required facility functions unless expressly authorized, qualified, and contracted.

• The contractor must not provide services outside the scope of licensure, competence, payer authorization, or the written agreement.

Service Expectations

• Maintain respectful, dependable, and clinically appropriate communication with residents, representatives, facility personnel, and the contracting organization.

• Notify the designated administrative contact of anticipated service interruptions, access concerns, documentation barriers, or material compliance issues.

• Protect company and facility property, credentials, records, and system access; promptly report suspected privacy or security incidents.

• Meet the performance, documentation, invoicing, credentialing, and quality standards stated in the independent contractor agreement and applicable service schedules.

Physical and Work Environment Considerations

Services may require travel between nursing facilities; movement through resident-care areas; prolonged sitting; use of computers and telehealth equipment; and interaction with residents who have complex medical, cognitive, psychiatric, or behavioral presentations. Any essential access requirement and reasonable modification will be addressed in accordance with applicable law and the written agreement.
