The Nurse Practitioner will provide family practice care to geriatric patients in an outpatient setting. This role focuses on delivering comprehensive patient assessments, developing treatment plans, managing medications, coordinating care, and supporting ongoing health and wellness through attentive clinical monitoring and documentation.
Responsibilities
- Conduct outpatient visits for geriatric patients.
- Assess patients and develop clinical diagnoses.
- Create individualized treatment plans.
- Manage medications and monitor treatment effectiveness.
- Coordinate care across providers and services.
- Perform health screenings and identify potential health concerns.
- Maintain accurate clinical documentation.
- Monitor patient progress and adjust care as appropriate.
Preferred Qualifications
- 2+ years of experience in advanced practice nursing.
- Master’s degree in Nursing or a related field.
